Heart Failure Management Programme
The cardiac rehabilitation service also offers a heart failure management programme at both James Cook and the Friarage.
This is an eight week programme of exercise, health information and relaxation. Patients can be referred directly from a cardiac rehabilitation assessment clinic or by the heart failure specialist nurse team or their consultant.
The exercise programme is designed to cater for the individual needs and ability of each participant. We aim to progress this exercise gradually over the duration of the course.
The health information package includes the following topics:
- Heart failure symptom monitoring and management
- Benefits of exercise
- Stress management
- Medicines advice
- Healthy eating for the heart
On completion of this programme options for long term exercise classes are available and will be discussed with patients on an individual basis.
Heart failure support group
For a number of years the heart failure support group has been had been held every three months. The support group offers ongoing help for patients who have been diagnosed with heart failure. This group provides the opportunity to keep up to date with any new advances in heart failure management, as well as providing ongoing support from experienced members of staff and other people who have heart failure.
Heart failure long term exercise programme
On completion of the hospital based heart failure exercise programme there are a number of community based exercise programmes that patients can progress on to. We currently have classes held in Middlesbrough and Redcar. These programmes will be discussed further with you during the hospital based programme and onwards referral will be made if you wish to attend.
