Who can be referred?

  • All new type 2 diabetics who need advice or help with glucose control or help with risk factor control
  • Type 2 diabetics who need advice / help with developing or existing complications
  • Diabetes secondary to steroids
  • Initiation of insulin
  • Initiation of new medication
  • Where patients refuse to attend secondary clinics
  • Existing type 1 diabetics with poor record attendance at secondary care clinics

Urgent referrals

  • Severe / recurrent / new hypoglycaemia


  • Suspected newly diagnosed type 1 diabetes
  • Type 1 or type 2 planning pregnancy or pregnant
  • Known type 1 diabetes age 16 – 24 years
  • Erectile dysfunction caused by diabetes

Referral process

Through the NHS e-referral service by completing the template at GP surgery

Referral by letter to specialist services admin, Third floor, One Life, Linthorpe Road, Middlesbrough, TS1 3QY.

Please include:

  • List of current medications and any drug allergies
  • Medical history and fresh lab data as above
  • If interpreter required please state this in the referral indicating which language
  • If transport is required this should be arranged by the referring agent
  • Person with diabetes will be offered an appointment to a clinic of their choice
