The MSK service can only accept referrals for people living in the Hambleton and Richmondshire area AND you should be registered with a GP Practice in Hambleton and Richmondshire. If you live outside this area please contact your GP, as a self-referral to the MSK Service would be rejected.
We cannot accept referrals for back pain that is located BELOW your waist. If the back pain is higher than your waist, then please continue with this form. If it is lower, please contact your GP surgery.
The information you tell us on this referral form will enable the form to be triaged by a clinical practitioner and your referral will then be directed to the service that is most appropriate to help you at your first appointment. It is therefore important you complete it as fully as possible.
If your GP surgery has a First Contact Physiotherapist you may benefit from an assessment and advice. Please note, they do not provide physiotherapy treatment. The Hambleton and Richmondshire musculoskeletal service only accepts referrals for arm and leg joints, any injuries to muscles, ligaments or soft tissue that is not getting better, neck pain and pain in the middle of your back.
We do not accept referrals for:
- Patients under the age of 18 years
- Lower back problems
- Patients who are already being treated in a hospital for the same problem
- Circulation problems/UlcerationsInflammatory conditions (for example rheumatoid arthritis)
- Neurological conditions
- Fractures or suspected fractures
- Dupytrens contractures
- Generally feeling unwell
- Multiple joint pain
- Suspected infection
- Suspected gout/pseudogout
- If you have any loss of bladder or bowel control or numbness/pins and needles in your saddle region please seek urgent medical attention
