  • A neurological physical disability and live in Middlesbrough (eg CP, ABI/pathology, SCI/pathology).
  • Adult CP in Middlesbrough
  • A package of care with funding for health needs (Middlesbrough or Redcar and Cleveland)

  • New stroke patients if under a neurologist and not an elderly care consultant

  • Adults requiring Intermediate Care rehabilitation including those with neurological diagnosis.
