What is rehabilitation?
Rehabilitation or rehab for short helps promote independence in activities of daily living. Rehab involves working with individuals, their families and carers to address underlying health conditions and symptoms effecting daily life.
The service can provide adaptations and support to benefit individuals, including provision of assistive equipment, deliver education and prescribe exercises to promote self-management and preserve independence.
Rehab requires full engagement and is not suitable for everyone therefore it is important to have this discussion with a professional prior to a referral to see if therapy input is going to be beneficial for you.

The team comprises of:
- Physiotherapists
- Occupational therapists
- Therapy assistant practitioners
- Therapy assistants
What do we do?
An initial assessment will be completed by one of the team. If you require further treatment, a therapy plan will be formulated with you to help you work towards your agreed therapy goals.
Specific goals will be set at the initial assessment, which will be personalised and meaningful to your rehabilitation. The team may provide regular home visits to ensure you are supported throughout the rehabilitation journey.
This service is not designed to provide long term rehabilitation and once you have achieved your goals, or rehabilitation with therapy has failed to progress, then you will be discharged.

- Provide rehabilitation in your own environment
- Support hospital discharges
- Reduce the risk of hospital admissions
Rehabilitation criteria
The team works with individuals following injury, illness or a decline in ability who require rehabilitation to help facilitate improvements in activities of daily living. Our service is for people who are:
- Over 18 years old
- Living in the Redcar and Cleveland or Middlesbrough catchment area
- Medically stable
- Unable to attend hospital appointments or requires an environmental assessment
