Bring your vaccine form
Please print off a COVID-19 vaccination form and bring your completed form with you so you can get your jab without delay!
Vaccine forms can also be completed at our drop-in clinics but you will need to bring:
Your payroll number
Your payroll number can be found by logging in to ESR and is on your payslip as assignment number. If you are not paid by the trust (such as junior doctors), you can leave the payroll number blank
Your NHS number
If you don’t know it, you can find it on the nhs.uk website
Roaming vaccination team
We will also have a team of roaming vaccinators on the wards who will be visiting high risk areas first.
Contact our roaming COVID-19 vaccinators on 07551 851607 to arrange department visits or if there is no one at the vaccination station.
