Stroke Early Supported Discharge (ESD) is a service that is offered to people who have had a new stroke, are well enough to leave hospital and who have been identified as appropriate for further specialist stroke therapy at home.
Therapy may include dietetics, occupational therapy, physiotherapy and speech and language therapy.
What does the stroke ESD team do?
The purpose of Stroke ESD is to help and support stroke patients to return home from hospital sooner and receive their therapy in the community.
This may include rehabilitation for:
- Mobility and movement.
- Communication and/or swallowing difficulties.
- Everyday activities such as washing and dressing or preparing meals and drinks.
- Emotional support.
- Care givers support, teaching and education.
- Secondary prevention and lifestyle advice.

Depending on your needs, the specialist therapists and assistants will visit you each week to work towards meeting your agreed goals.

How long will I receive this service?
Stroke ESD is available for up to 6 weeks, to assist you to continue your therapy at home.
The number of appointments each week will depend on your progress and will be decided by the therapy team.
Appointments will be given for either morning or afternoon, no specific time will be given as your therapists will be travelling from different locations.
What we expect from you
To help you benefit the most from your therapy, we expect the following:
- You are willing to participate in a therapy programme.
- Participation in a self-management programme between visits.
- You refrain from smoking in the property immediately prior to and during any visits.
- You are available for your appointment. Please be aware that if you miss two appointments, you will be discharged from ESD.
