Referral to our occupational therapy services can be made by any professional who is working with a child or young person registered with a GP within the Middlesbrough, Redcar and Cleveland area.
This includes:
- Teachers
- Consultants
- GPs
- Other health professionals
We do not accept self referrals.
We ask that staff referring obtains consent from the parent/carer of the child before the referral is sent.
Professionals completing the referral form, should ensure it is completed in full and that the details are up to date and correct.
Once complete, the referral form should be be returned via post or email
