All patients who are registered with a GP in the Middlesbrough, Redcar and Cleveland area can be referred by their doctor, any healthcare professional or social care worker.
Many of our patients come to us after treatments in hospital and are referred on by their consultant for their continuing rehabilitation.
How to self refer
If you are registered with a GP in the Middlesbrough, Redcar and Cleveland area, and are aged over 18 years you can self refer via the community outpatient physiotherapy self-referral form.

If you do feel that you require an urgent appointment, this must be authorised by your GP and they will need to initiate an appropriate referral.
We do advise patients to request an appointment with the FCP in their GP Practice prior to self referring
