This is a non-urgent service. Any care requiring urgent review and/or treatment should be managed through existing pathways.
Referrals will be accepted until 12 months after birth.
Women and birthing people can be re-assessed at any time during the perinatal period.
After the perinatal period please follow existing referral pathways, such as via your GP.
Perinatal pelvic health service (PPHS) referral
You are about to complete a referral to the Perinatal Pelvic Health Team (PPHT). The PPHT accept referrals for the following pelvic health problems;
- leaking of urine (wee) and/or stool (poo)
- pelvic organ prolapse (bulging or swelling)
- third or fourth degree tears
- postpartum sexual pain.
Please only complete this form if you are booked to give birth at, or received your labour care from South Tees Hospitals NHS Foundation Trust.

Is there a limit to accessing the service?
Women and birthing people will be able to self-refer at any point throughout their pregnancy journey, or up to 12 months after their birthing experience.
- Urinary tract infection- (pain or a burning sensation when weeing, urine that looks cloudy, darker or stronger smelling than usual.
- Wound infection or breakdown (including an increase in pain, new bleeding or pus-like discharge, or feeling unwell)
- If you notice blood in your urine, please contact your GP
If you are experiencing the following symptoms, please go to A&E
- Urinary retention (a sudden inability to pass urine, usually with some pain or discomfort in your bladder)
- a loss of sensation when passing urine and/or opening your bowels

How can I self-refer to the perinatal pelvic health team?
Self-referral to the service can be done by completing this form with all the relevant information.
If your symptoms change at any point, or you do not feel you have received the care you need, you can re-refer at any time up to 12 months post-natal or you can speak to your midwife, GP or HV.
Storing your information
Your self-assessment form will be sent securely to the Physiotherapy Department. This website will not store your personal information. Providers will delete the self-assessment forms within 30 days of completion, unless you subsequently go on to access an appointment based on the information given. In this case, your form will be uploaded to your trust medical record. If you prefer not to have your information stored in this way, please access services via your GP.
