Inclusion criteria for community therapy
Do
Over 18
Living in the Redcar and Cleveland or Middlesbrough catchment area
Medically stable
Unable to attend hospital appointments or requires an environmental assessment
Exclusion criteria
Don’t
Under the age of 18
Medically unstable
Manual handling assessment only
Equipment provision only
Bed provision only
Pressure care only
Acute respiratory interventions
Unable to engage in rehabilitation due to pain or cognition
Wheelchair or specialist seating only
If mental health is primary condition
If any of the above issues are raised, please consider referral to alternate services including (contact numbers can be found below):
- Adult social care – for manual handling, specialist seating and equipment provision
- Community nurses – for bed provision and pressure relieving equipment
- Urgent community response – for respiratory interventions which can be managed in the community (non-life threatening
- Wheelchair services – wheelchair assessments
Referral process
Community therapies
If the service user has rehabilitation goals and you wish to submit a referral however do not have access to the new referral form, please email [email protected] to request a copy of the form.
Social care
If the service user does not have rehabilitation needs however requires a moving and handling assessment / environmental assessment / equipment provision, please fill in the social care referral form and email it to the relevant service.
Alternatively, if you do not have access to the referral form, please use the details below to request a copy of the referral form.
