The service predominantly provides diabetes care, general wound care (including pressure ulcer and leg ulcer management), administration of medications via a variety of routes, intravenous antibiotics, palliative and end of life care, diagnostic testing and phlebotomy, catheter care and complex patient management.
Care is provided in patients’ homes, including residential homes, 24 hours a day, seven days a week.
Alongside the core community nursing service, unplanned care is provided as well as a rapid response and out of hours services to facilitate early discharge from hospital and to prevent hospital admissions.
Core community nursing hours including unplanned work and rapid response is currently provided between 8am and 11pm, seven days per week. Out of hours services are provided between 10pm and 8am.
