The fast response team accepts referrals from other health professionals and social workers and consists of qualified nurses and support workers who aim to support patients to maintain their independence either in their own home or in a community bed.
The service does not provide respite care, continuing or long-term care, but provides a time limited service for 72 hours. It provides guidance and support in a crisis situation by providing a holistic assessment of a patient’s needs at the time they are referred.
The team will then discuss this assessment with the patient, outlining any treatment plan and support it can put in place for the individual.
Patients who require longer-term care can be referred to appropriate services by the team if they consent.

Who is this service for?
This service is available to adults over the age of 18 who live within the Hambleton and Richmondshire area and are registered with a GP there.
This locality covers an area of approximately 1,000 square miles. The team expects patients to provide relevant information to ensure an assessment of their needs can be carried out accurately.
